Here are four possible solutions:
First, you have to make up your mind if you actually need help. Many people put off getting help for their neck and shoulder pain for as long as six months because they think it will “just go away with time.” However, after that amount of time has passed, the pain hasn’t gone away and, in many cases, has become worse.
Doing the correct exercises is the next step. Performing the appropriate series of progressive exercises is one of the best strategies to alleviate pain in the neck and/or shoulders. The right workouts will help you feel better faster, regain your mobility more completely, and prevent future problems.
Avoid sitting or sleeping in awkward positions. How often do you wake up with a stiff neck? Maybe you spent all day sitting and now your shoulders hurt. Due to the fact that sitting awkwardly and sleeping in an unnatural position are two of the worst things you can do to aggravate neck and/or shoulder pain. In addition to receiving hands-on treatment, it would be beneficial if you were also taught the appropriate strengthening exercises for better posture.

Common presentations include cervical strain from prolonged desk posture, rotator cuff injuries from sport or overuse, thoracic outlet syndrome, and cervicogenic headaches where neck dysfunction drives head pain. Many patients have overlapping neck and shoulder involvement that requires addressing both areas together rather than in isolation.
PT for neck and shoulder pain includes manual therapy to restore joint mobility, targeted strengthening to rebuild stability, and movement retraining to address the posture and mechanics fueling the problem. The goal is durable resolution — not temporary pain relief that returns as soon as you go back to normal activity.
Neck and shoulder pain often share root causes. Nerves exiting the cervical spine refer pain and dysfunction into the shoulder, and poor thoracic mobility affects both regions. A skilled PT assesses the entire chain rather than treating each in isolation — which is why treating just the shoulder often fails when the neck is the actual driver.
In most cases, no. Your PT can assess functional limitations, tissue sensitivity, and movement quality without imaging, and treatment can begin immediately. If imaging becomes relevant — for example, to rule out a structural issue that changes the treatment approach — your clinician will advise accordingly.
Most patients see meaningful progress in 4–8 sessions. Acute or new-onset pain often resolves faster than longstanding or post-surgical cases. Your timeline will be clearer after your first evaluation, and your clinician will track specific markers to assess progress between sessions.
